On your first visit we will complete a comprehensive history, examination, and treatment of the pain, injury, or ailment in concern. This initial exam will take 40 minutes to 1 hour, so please allow adequate time.
Please arrive 10 minutes prior to your appointment time and bring any insurance or referral information that is necessary
If you have any MRI's, X-ray's, Labs, or any diagnostic testing, please bring the original films or CD if possible.
Our New Patient forms are available electronically! After contacting our office as a new patient, we will create your online portal. If you have an active email address the forms will be sent privately to your inbox. Please fill out the information prior to your appointment time and simply submit the form online. There is no need to print any of the information as it is linked to our electronic software.
If you do not have an email or are not able to complete the forms before hand, do not worry as we will help you to complete this at the office. Please arrive with enough time to complete the paper work before the visit.
